Permissions Form

To request permission (after reading the rules!) to use all or part of Jan's Illustrated Computer Literacy 101, fill out and submit the form below.

My Reply:
You should get a response from me in a day or so. If you don't hear back, there may be a problem, such as:

Your email address was mis-typed in the form. (Whoops!)
Your email account is full - too many messages stored on the email server, includes the Trash folder.
Your email account has been closed.
You have restricted incoming mail to people already in your address book or from only certain email servers.

If you have a problem, email me directly, instead of submitting the form again. Include a second email address from a different service if at all possible.

If you do not yet have an email address, you can sign up easily for a free email account with services like HotMail.com and Yahoo.com.

Front Page or Server Errors:
If you get an error message, please email me directly and include the text of the error message. You do not need to submit the form again right away. The results may be correctly stored in my database.

If you see a password dialog, then the server is hallucinating. Please email me directly, otherwise I will not know that there is a problem!
